MISSION STATEMENT

Our mission at Blessed Hands Barbering Academy is to offer the best barbering, health, safety, and small business operations training so students can lead sustainable lives in the barbering industry and career. Blessed Hands Barber Academy, we prioritize our students to pass all Arizona State Board requirements, including all theory and practical objectives. Still, we want to prepare our students with all the modern practical skills to strive and prosper beyond obtaining their Barber License in this career. We aim to prepare our students for employment in this booming career workforce by teaching them the methods and skill sets to succeed in their work or establish their own businesses.

PROGRAMS

Blessed Hands Barber Academy offers the following programs. BARBERING(1200 hours) and BARBER/COSMETOLOGY CROSSOVER COURSE (200 hours).

EDUCATIONAL OBJECTIVES

At Blessed Hands Barbering Academy, we aim to effectively prepare our students to pass the Arizona State Board of Barbering and Cosmetology examination, which will license them to practice barbering in Arizona. At BHBA, we strive to teach our students the most professional and up-to-date practices in Barbering and managing barber shops. Our Barbering Academy instructors will be teaching in a step-by-step method during our theory and practical hours to ensure that students can learn effectively and engage in one-on-one conversations with students.

INSTRUCTIONAL LANGUAGE– Instruction is in English, and the instructor is bilingual in Spanish.

FACILITY AND EQUIPMENT

The academy location is 973 S Main Street, across from Kentucky Fried Chicken. The campus occupies approximately 1,200 square feet of floor space. There is ample free parking. Blessed Hands Barbering Academy consists of, a classroom, a break area, and a Barbering practice area. All space is entirely air-conditioned. Our instructors deliver education in the classroom through lectures, audio and video presentations, and demonstrations in the student clinic. The practical classroom is equipped with student stations, computers with internet access, and treatment tables for the student to practice and perform the techniques to the public under the direct supervision of a licensed instructor.

ADMISSION POLICY

Proof that the applicant is at least 16 years of age.

Proof that the applicant has at least two years of high school education or its equivalent. Acceptable proof includes an official transcript from the high school attended or a copy of a high school diploma or GED. If not a U.S citizen

A non-refundable $100 application fee is due at registration

Documentation specified under A.R.S. § 41-1080(A) that the applicant’s presence in the U.S. is authorized under federal law. EXAMPLE: Passport, Driver’s license, U.S Birth Certificate, etc.

ADMISSION PROCEDURE

The student arranges for a personal interview with the Director/ instructor. During the interview, a tour will be provided, and questions will be answered.

The student will complete the application.

The student will provide all necessary documentation from our admission policy.

The student will be assigned a date and time to begin their instruction.

The student completes their enrollment agreement, tenders payment, and makes payments.

INSTRUCTIONAL METHOD

The clock-hour education is provided through a sequential set of learning steps that address specific tasks necessary for the state board preparation, graduation, and entry-level job skills. Clinic equipment, implements, and products are comparable to those used in the industry. Each student will receive instruction on performing practical, creative, and productive career-oriented activities. The course is presented through comprehensive lesson plans that reflect effective educational methods. Subjects are presented utilizing interactive lectures, demonstrations, cooperative learning, labs, student activities, and student participation. Audio-visual aids, guest speakers, field trips, projects, activities, and other related learning methods are used in the course.

BARBER/ COSMO CROSSOVER PROGRAM

Must have a valid Arizona State Cosmetology license before enrollment.

NON-DISCRIMINATION POLICY

Blessed Hands Barber Academy does not discriminate based on race, color, religion, sex, disability, financial status, age, area of origin, or residence in its admission, staffing, or instructor.

RE-ENTRY (RE-ENROLLMENT POLICY)

Students who withdraw from Blessed Hands Barber Academy may re-enter the same study without the loss of credits for prior hours and operations earned during previous enrollment if re-entry occurs within 180 days of the last day of attendance. A student terminated for behavior reasons may not be eligible for re-entry. Students who re-enter more than 180 days after the last day of attendance will have their transcripts evaluated for the number of credits and services they will receive credit for, and review and transferring hours will be at the sole discretion of the school’s Director. At the time of re-entry, another $100 registration fee.

TRANSFER STUDENTS

Accepting any transferring hours and services is at the discretion of Blessed Hands Barbering Academy. Transfer students will receive credit for previous training from an accredited barber school with proper documentation of hours, services, and tests, and after being evaluated in theoretical and practical abilities by the Barber Academy Instructor. At most, 75% of the program credits will be transferred into the school from another institution. The barber program’s educational costs will be adjusted. For course credits to be considered, students must request an official transcript be sent directly to Blessed Hands Barber Academy. In addition, students may be required to provide catalogs and other relevant documents regarding courses to be considered. All systems must have a “B” or higher on the transcript from the awarding institution. The competencies of studies submitted for transfer credit must be comparable to those at the Barber Academy for the credit to be awarded. The transfer from The Blessed Hands Barber Academy to another institution is at the discretion of the accepting institution; The Blessed Hands Barber Academy cannot guarantee the transferability of credits earned at our school. It is the student’s responsibility to confirm whether the clock hours & services earned at the Barber Academy will be accepted by another institution of the student’s choice.

NOTICE OF ADMISSION ACCEPTANCE, DENIED APPLICANTS, AND WAITLIST

Students will be notified of their acceptance by phone, email, or in person within 14 days of their admissions interview. Students who are denied approval may reapply. In the event of a waitlist for enrollment, students will be enrolled on a first-come, first-served basis from when all admissions requirements have been met.

CLASS SCHEDULE

Blessed Hands Barber Academy class hours are Monday- Friday from 9 am – 5 pm.

Closed Saturday and Sunday and legal Holidays, including Day/Thanksgiving Day/Labor /Memorial Day/Christmas Day/Independence Day/New Year’s Day. The institution’s owner may declare additional holidays, which will be announced at the appropriate time with proper notice.

ADMINISTRATION HOURS

CLASS START DATES

The barber academy has a continuous enrollment policy with new classes starting every week, Which means that students can enroll anytime during the year because instruction occurs in teaching modules. Enrollments are restricted to maintain a low student-to-instructor ratio.

ORIENTATION DAY

Students must attend an orientation before beginning their program. No clock hours are earned at orientation. Orientation will review school policies and regulations as well as course outlines.

TUITION AND FEES

1)Barber Program:

• 1,200 Barbering Instructional Hours: $15,000.00

• Supplies/ Textbook and workbooks/BARBER KIT: $1635

• Registration Fee: $100.00 (Non-Refundable)

TOTAL CHARGES If paid in Cash Amount: $15,000.00

TOTAL CHARGES If paid in Payments: $5,000 down payment and eight monthly payments of $1,250.00. All fees are due before graduating and receiving the certificate of completion.

2) Cosmo Crossover Program:

200- Barbering Instructional Hours: $ 10,000

Supplies/ Textbook and workbooks/BARBER KIT:$1635

Registration Fee: $100 (Non- Refundable)

TOTAL CHARGES If paid in full: $10,000

TOTAL CHARGES If paid in payments: $5000.00 down payment and two payments of $2500.00. Due before graduating and receiving the certificate of completion.

PAYMENT SCHEDULE: 1st of each month after initial payment or deposit.

OTHER FEES The Arizona State Examination for Barbering costs $143.00, subject to change per the State of Arizona

TUITION FOR TRANSFER STUDENTS

The tuition portion of a transfer student’s fees, excluding any books or kits required, will be computed on a pro-rata basis of the number of hours they are contracting of the current tuition rate. The academy will determine at its sole discretion if transfer students must purchase new books and kits on a case-by-case basis.

METHOD OF PAYMENT

Students are expected to make all monthly payments in cash, money order, or check; there will be a $35.00 fee for returned checks and a $35.00 late fee when tuition is not paid by the 3rd business day. All school charges must be paid in full before graduation.

TARDINESS

Students are expected to arrive to class on time and prepared to learn, as tardiness causes the student to miss valuable instruction and disrupt the class. A student arriving 10 minutes after education has begun is considered tardy. Any student late to class three (3) times will be recorded for one unexcused absence and counseled by the instructor. Any further tardiness will result in the student being required to make up hours and work missed at a scheduled time agreed upon by the instructor.

ATTENDANCE POLICY

Attendance must be maintained at an average of 80% of the scheduled attendance if the student is expected to complete the course of study within the period stated in the enrollment agreement. Students must make up for the lessons and exams missed due to absence. If the student is absent for five consecutive days, the school will withdraw them. The instructor in charge will review excessive tardiness or absences with the student to determine possible corrective action to the issue on hand. Students who cannot attend class should notify the Barber Academy 5 days before the absence. Having that the program is set up for a specific time frame, vacation time should be discussed and approved at the initial interview or 30 days of any vacation time that may be requested. At most, five consecutive days may be granted as vacation time due to our program time frame requirement. Three (3) unexcused absences within 30 days will result in termination of the program with no refund returned.

BARBER CURRICULUM & SCHEDULE (1200 HOURS)

One thousand two hundred hours are required to take the Barbering and Cosmetology state board test. Break down of these required hours includes 950 practical instructional hours that will be made up of the practice of Haircutting,Shaving, Shampooing, Facials and Massage Treatments, Scalp Treatments, Hair Treatments, Basic Hair Styling, Coloring, Hair Care, Skin Products, Hygiene,Sanitation. Two hundred fifty hours are required on the theory of Barbering, which includes The History of barbering (15 HRS), Professional image (15 HRS), Bacteriology (15 HRS), Infection Control (15 HRS), Safe work practices(15 HRS), Implements tools and equipment (15 HRS), Anatomy and Physiology (10 HRS), Chemistry (10 HRS), Electricity and light therapy(10 HRS), Salesmanship (10 HRS), Arizona State board rules and regulations (10 HRS), Properties and disorders of the skin (10 HRS), Hair and scalp (10 HRS), Treatments of the hair and scalp(10 HRS), men’s facial and massages (10 HRS), Shaving and facial hair design (10 HRS), Men and women hair cutting and styling (10 HRS), Hairpieces (10HRS), Chemical texture services(10HRS), Hair coloring and lightening(10 HRS), Professional ethics(15 HRS)and Barbershop management (15 HRS).

GRADUATION REQUIREMENTS

Attend all required hours of the barber program (1200 hours) and (200 hours) for the cosmetology crossover program as required by the State Board of Barbering and Cosmetology, with maximum hours of 1500 for the Barber program (125% of the proposed program timeframe). Maximum of 250 (hours) for cosmetology crossover program. Complete the program’s requirements (tests, practical hours, client services). In compliance with Arizona statute, a student, upon completing hours of a course of study and passing the final exam, is awarded a diploma verifying program completion. Graduating does not give you a barber license; students must pass the State Board Barber Exam to receive their license. A student graduates from this barber program after completing 1200 or 200 hours for the Cosmo crossover training program and passes their final exam with a satisfactory grade (80% or higher). Satisfy all financial obligations to the institution. Obtaining your official Arizona license requires students to pay for the state board’s examination and licensing fees and pass with an 80% on both practical and written exams that the Arizona Barbering and Cosmetology Board requires.

GRADING FACTORS

To maintain a satisfactory academic status, students must maintain a “B+” (85%) average on all chapter and subject tests. The grading factors are as follows. Grade Scale alphabetical, 90-100% (A) 89-80% (B) 79-70% (C) 69-60% (D) 59 or below 60% (F).

EVALUATION PERIOD

Students are evaluated for Satisfactory Academic Progress as follows: Barber 100, 200, 300, 600, 900, and 11,000 clocked (actual) hours.

Transfer Students- Midpoint of the contracted hours or the established evaluation periods, whichever comes first. Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The frequency of assessments ensures that students have had at least one evaluation by the midpoint in the course.

BARBER PROGRAM TIME FRAME

Full Time- 30 weeks Total. It would require completing 40 hours a week, for a total of 1200 hours.

Part-time- 60 weeks Total. It would require completing 20 hours a week, for a total of 1200 hours.

A student may clock in up to 40 hours a week.

COSMETOLOGY TO BARBER CROSSOVER PROGRAM TIME FRAME

Full Time- 5 weeks. It would require completing 40 hours a week for 200 hours.

Part time-10 weeks. Which would require completing 20 hours a week for a total of 200

A student may clock up to 40 hours a week.

EXTRA-INSTRUCTIONAL CHARGES POLICY

Each program has been scheduled for completion within an allotted time frame. A grace period of approximately 25 percent has been added to the calculated completion date for each program. The institution has reserved space, equipment, and licensed instructors for each student and program. If a student does not graduate within the contract period, additional training will be billed at $250 per week, or any part thereof, payable in advance until graduation. Students can only clock in once applicable weekly payments are made. Students can officially withdraw from the school at any time by giving written notification to the president. Unofficial withdrawal can occur when the student fails to attend classes and has made no contact with the school administration. A grade of “W” will be assigned upon withdrawal determination. Upon withdrawal, official or unofficial, the school’s refund policy will apply, and arrangements must be made to pay any balance due to the Academy. Students cannot re-enroll in the Barber Academy with an outstanding balance. Any monies due to the student at the time of withdrawal, official or unofficial, will be refunded to the student within 15 days of the determination date. No refund will be returned if a withdrawal occurs after completing more than 50% of the program’s schedule. There will be a $300 withdrawal or cancellation fee.

ACADEMIES STANDARDS FOR STUDENT ACHIEVEMENT

Blessed Hands Barber Academy students are measured by class time, practice time, and exam results. Students record their attendance through the use of a manual time clock. Students receive credit for completing a task/operation/project after an instructor verifies each action.

EMPLOYMENT ASSISTANCE/PROFESSIONAL DEVELOPMENT

Students will learn about communication skills, professional ethics, customer service, resume writing, job search skills, preparing for an interview, salesmanship, decorum record keeping, client service, record cards, essential tax responsibilities related to independent contractors, booth renters, employees and employers, marketing and client retention as well as professional ethics and barber shop management.

FACULTY AND QUALIFICATIONS

A licensed Barber Instructor in Arizona, Ramses Alan Fletes Uribe, provides all instruction.

GENERAL TERMS

1. Diplomas will be issued after the completion of the entire program. The Student must achieve a passing grade average to receive a diploma. Students will be permitted a re-test for any grade below passing. The Instructor may grant make-up time for extenuating circumstances.

2. All course schedules are subject to change in starting and completing dates. Students will be duly notified.

3. Blessed Hands Barber Academy reserves the right to change or modify the program content, equipment, staff, materials, and organization if necessary. Such changes may be required to keep pace with technological advances and to improve teaching methods. In no event will any changes diminish the competency of any program or result in tuition changes for current attending students.

SKILLS AND COMPETENCIES TO BE ACQUIRED BY THE STUDENT

After the program, the student will have acquired the following skills. Apply scalp and hair treatments, including the use of therapeutic massage. Prepare the client’s hair for shaving, assess the condition of the client’s skin, perform shaving techniques, apply after-shave anti-septic, massage the client’s face, and roll cream. Use and handle all barbering-related tools such as clippers, trimmers, scissors, razors, curling irons, blow driers, combs, and brushes.

COSMETOLOGY CROSSOVER TO BARBERING PROGRAM (200 HOURS)

Our cosmetology crossover program prepares students who have completed an Arizona-approved cosmetology program or are licensed cosmetologists for licensing as a Barber. The Cosmetology to Barbering crossover program requires 200 hours of comprehensive training in barbering services. Courses in Cosmetology to Barbering Crossover program focus on theoretical foundation, practical manipulative skills, business knowledge, client services, professional and ethical training, and preparation.

COSMETOLOGY CROSSOVER TO BARBERING PROGRAM OBJECTIVE

Our goal at BHBA is to provide our graduates with the required training, knowledge, and skills to obtain their barber license from the state board. We also want to properly teach our students the skills needed to work in a barber shop in the compilation of our program. Skills that are required to be successful in the barbering profession include regular men’s haircuts, styled men’s haircuts, facials, and shaving with a straight razor and all the up-to-date procedures to perform at top levels in the industry, as well as all sanitary and disinfecting procedures required by the state board.

COSMETOLOGY CROSSOVER TO BARBERING PROGRAM GOALS

Successfully master training requirements to instruct by demonstration, lecture, classroom participation, or examination.

To develop professional skills and attitudes in the interest of the student’s learning environment.

To graduate from this institution with those skills to help secure entry-level employment at a minimum.

To successfully enforce the knowledge of the laws and rules regulating Arizona.

To successfully enforce the knowledge of sanitation and sterilization as required by the state of Arizona.

Teach and deliver knowledge of the general theory to barbering

Teach business management techniques, which are common in the barber industry.

Teach the necessary skills to perform all practical barbering procedures

Teach and practice the proper use of implements used in barbering

Acknowledge all the methods and terminology used when performing barbering services.

Explain the physical and chemical actions during chemical texture services and demonstrate the proper procedures.

EXAMS AND GRADING

The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic(qualitative) learning. Students are assigned both academic(written exams) and several practical evaluations. Academic learning is evaluated during each unit of study using class quizzes, written exams, and practical tests. Practical skills are assessed according to course rubrics adopted by the school and provided to students. Students must maintain a grade average of 75% to maintain satisfactory academic progress. Students may make up for failed or missed tests and incomplete assignments following the school’s published policy for scheduling and grading make-up work, Incompletes, and repetitions. The combined average of all practical and written examinations determines a student’s grade point average. The

RULES AND CONDUCT POLICY

The rules and regulations of the Barber Academy are designed to help the students learn professional habits at the start of their professional careers. All students must comply with the sanitary requirements and not be unruly or interfere with other students. The students shall dress professionally. Students must comply with all laws (federal and state), particularly laws relating to barbering. Students may be held liable for damages to the college and its equipment. Institutions’ rules and regulations are as follows:

1. Must follow dress code: Students must always wear smocks. Belts must be worn; no sagging pants. Students dressed inappropriately will not be permitted in class. Students will be sent home if they do not have the proper tools, and will be counted as an unexcused absence.

2. Attend class on time; no one is admitted after 9:10 a.m.

3. Student is 100% responsible for their time card; there is zero tolerance for clocking in for another student.

4. Must have a doctor’s note to be excused from absence.

5. Must return on time from breaks and lunches as part of your attendance. Late entrance will only be permitted if communicated with the instructor.

6. Must bring barber workbook, and barber supplies daily. Students will be sent home if they do not have the proper tools, and will be counted as an unexcused absence.

7. Cell phones must be on vibrate and put away in a designated area.

8. Zero tolerance for disrespectful behavior or insubordination of instructors or students. This behavior will result in immediate expulsion.

9. Theft of any kind shall result in immediate termination. Lying, cheating, and stealing of any kind will not be tolerated.

10. All students must conduct themselves professionally at all times. Interaction with administration, faculty, clients, and fellow students should be courteous and professional. Unprofessional behavior may result in your termination.

11. Confrontational or threatening behavior towards fellow students, faculty, staff, or clients shall result in immediate termination.

12. All sanitation, sterilization, and cleanliness rules must always be followed. Your stations and work tools must be sanitized and sterilized correctly before usage.

13. Food or Drinks are only allowed in designated areas.

14. Smoking is prohibited in school facilities and only allowed in designated areas outside the Barber Academy.

15. Students cannot play loud music or wear headphones during class and while servicing customers

16. Absolutely no gambling or card playing allowed on school premises

17. Daily attendance is mandatory. Failure to attend the Barber Academy in accordance with attendance policy will result in a written or verbal warning, followed by an academic warning and then termination. Please notify the school in advance if you will be absent or late.

18. All monies owed must be paid before graduation unless the school owner has made and approved prior arrangements.

19. Hazing of any sort will not be tolerated, nor does the Barber Academy condone such behavior. Students found hazing will automatically be terminated.

20. No use or sale of illegal DRUGS is allowed on school property, including Marihuana. These actions include suspension and even termination. All evidence of illegal drug or alcohol activity will be reported to the Law Enforcement Agencies.

A student is subject to TERMINATION for the following (but not limited to):

Failure to maintain satisfactory progress

Failure to comply with the Barber Academy’s attendance policy

Failure to comply with the Barber Academy’s student conduct policy.

Failure to meet all financial obligations to the Barber Academy.

Violates any of the conditions outlined in the signed enrollment agreement.

ACADEMIC PROGRESS STATUS

Students meeting the minimum academic requirements and attendance at the evaluation point are considered to be making satisfactory academic progress until the next evaluation. Blessed Hands Barber Academy does not allow for the status of probation.

RE-ESTABLISHMENT OF PROGRESS

Students need to meet satisfactory academic evaluations to be completed by the school, and students will be notified of their assessment results either by email(electronically) or in person (paper).

ACCESS TO SATISFACTORY ACADEMIC PROGRESS

A hard copy of each satisfactory Academic progress evaluation is maintained in the student’s file.

INTERRUPTIONS, LEAVE OF ABSENCE, AND WITHDRAWS

If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school with the same progress status as before the leave of absence. Hours elapsed during a leave of absence will extend the student’s contract period and maximum time frame by the same number of days taken in the absence. They will not be included in the student’s cumulative attendance percentage calculation. Students who withdraw before course completion and wish to re-enroll will return with the same satisfactory academic progress status as the withdrawal time.

INCOMPLETES

Incompletes, withdrawals, and repetitions do not apply the institution and do not affect the institute’s satisfactory academic progress standards if the institution has no such items or policies.

RECORDS AND ACADEMIC TRANSCRIPTS

Student records will remain on site for six years, and transcripts are kept permanently. Students who need proof of training or a transcript of their activity may contact the school office and request the form desired. Transcripts should be provided to students who have ledger balances. Transcripts will only be released to the student upon receipt of a written request bearing the student’s live signature.

Blessed Hands Barber Academy library contains reference sources and current material on new services. Currently, for students to review and be able to check out from the library, which is a book shelve in the school office, are listed below:

HOUSING

Blessed Hands Barber Academy has no dormitory facilities; the program is non-residential. We have no responsibility to find or assist a student in finding housing.

ARIZONA STATE LICENSURE REQUIREMENTS

To qualify for admittance for the Arizona State Barber exam, an applicant must have provided to the State Exam Board the proper forms, paid the required fee, be not less than 16 years of age, have completed the 10th grade in the public schools of this state or its equivalent, is not subject to denial under Section 480 of the Barbering and Cosmetology Act, and has completed 1200 hours or 200 hours for the cosmo crossover program in the course of Barbering from a school approved by the board.

COLLECTION POLICY

Blessed Hands Barber Academy collection procedures reflect ethical business practices, the name of its accreditor will not be used in any collection efforts. All correspondence regarding cancelation and settlement by or on behalf of the academy will acknowledge the existence of the withdrawal and settlement policy. If promissory notes or contracts are sold or discounted to a third party must comply with the Institutional Refund policy and the Collection Policy of the school.

BARBER PRE-REQUISITES AND PHYSICAL DEMANDS

Prospective students should be aware of the physical demands of a barber in this industry.

Students with criminal convictions are encouraged to contact the Arizona State Board of Barbers to see If their conviction would exclude them from taking the state licensure exam before enrollment.

Occupations in the barber industry generally require prolonged standing or sitting and constant use of the upper torso, shoulder, arms, wrist and hands, upper back, and neck. Prospective students should have finger dexterity and a sense of form and artistry, enjoy dealing with the public and be able to follow clients’ directions, and always have compassion and patience for people. A student must be physically capable of performing all required activities at the school and comply with all policies and procedures to work as a Licensed barber. Individuals with allergies or other sensitivities may have reactions to typical chemical products used in the barber industry. Exposure to chemicals used in the barber industry may cause cancer and congenital disabilities, or other reproductive harm to you and your physician regarding these issues before enrolling and signing an enrollment agreement. Generally, the professional in the barbering field of study requires a great deal of standing while working on patron, with shoulders, arm, and hand muscle movements. A person must consider their physical limitations when choosing a career involving extensive physical demands.

REFUND POLICY

Should a student’s enrollment be terminated, withdrawn, or canceled for any reason, all refunds will be made according to the following refund schedule:

1. Cancellation can be made in person, by electronic mail, by Certified Mail, or by termination.

2. All monies will be refunded if the school does not accept the applicant or if the student cancels within three (3) business days after signing the enrollment agreement and making the initial payment.

3. Cancellation after the third (3rd) Business Day, but before the first class, results in a refund of all monies paid, except the application fee ($100.00).

4. Cancellation after completing more than 50% of the program will result in no refund.

5. Termination Date: In calculating the refund due to a student, the student’s last date of actual attendance is used unless an earlier written notice is received.

6. Refunds will be made within 15 days of termination of students’ enrollment or receipt of a Cancellation Notice from the student.

7. The cost of the barber kit, textbooks, and workbooks are non-refundable ounces received by the student

8. In case of illness or disability accident. Death in the immediate family, or other circumstances beyond the student’s control, the school attempts to make a reasonable settlement for both.

9. There will be a $300 cancellation or withdrawal fee.

GRIEVANCE POLICY/ PROCEDURE

Students are expected to address any disagreements or conflicts or possible records in student records directly with the individual involved in person with a written document outlining the complaint and communication. After this, if there is no satisfactory resolution, the student may set an appointment to see the school’s director. All communications regarding the complaint must be in writing, and all meetings and communications will be documented in the student files. The grievance policy is as follows: 1. Complaints regarding students or Blessed Hands Barber Academy staff must be made within one week of the issue. 2. If the complaint cannot be resolved informally (verbally), the student shall write up the details and submit them to the school director (within 30 days of the incident), who will research the issue and respond with a resolution. Please allow the school director up to 10 business days to respond to written complaints in writing. If such a response is unsatisfactory, the student may contact the Arizona State Barber and Cosmetology Board, 1740 W Adams St, Phoenix, Arizona 85007, (602) 542-4498.

BANKRUPTCY HISTORY

Blessed Hands Barber Academy LLC does not have a pending petition in bankruptcy, is not operating as a debtor in possession, has not filed a petition within the preceding five years, or has had a petition in bankruptcy filed against it within the preceding five years that resulted in reorganization under chapter 11 of the United States Bankruptcy Code ( 11 U.S.C. SEC 1101 et seq).
